Documents Needed for Bankruptcy

1. Driver’s license;

2. Social security card;

3. Tax returns, both state and federal, for the two most recent tax years, including all schedules and attachments;

4. Pay advices/paystubs relative to any payments received from employers within 7 months of the filing date. If self-employed, need monthly profit & loss statements; If receiving non-employment income (unemployment, child support, Social Security, retirement), also need records of what you have received. We need records for you and your spouse, even if your spouse is not filing.

5. Life insurance policy declaration page (or other similar documentation) for each policy showing the type of policy, the owner, the insured, the beneficiary(ies), the amount of cash value (if any), and the amount of any outstanding loan;

6. Most recent 401(k), IRA, 529 education account statements; Statements for all other investment accounts.

7. Most recent mortgage statement;

8. Registration cards for each automobile or other vehicle owned or, if owned free and clear, vehicle titles; and 9. Bank statements for all accounts for the 90 days preceding and including the filing date. Also include statements for Paypal, Venmo, and other nontraditional financial accounts.

10. Copies of any lawsuits filed in the past year and all unpaid judgments within the past 20 years.
